Position Purpose:

Utilizing a strengths-based approach, the Case Manager will perform some or all of the following duties:

·       Assists program by providing case management

·       Assists with housing/transitional living

·       Assists with childcare needs

·       Assists with transportation needs

·       Assists with Medicaid and food stamp enrollment

·       Assists with any referral-based services as needed

·       Assist in emergency assistance and stabilization 

·       Motivates students through difficult life changes

·       Manage all wraparound services (i.e. dental, legal, budgeting, etc)

·       Complete intakes prior to student involvement in school

·       Complete accountability plans with the students to remove barriers to service

·       Checks in weekly with students

·       Acts as a court advocate

·       Drug tests students

·       Assists with student recruitment and marketing of the program

·       Participates in student evaluations in the program

·       Participation in multi-disciplinary team meetings

·       Communication with other departments including employment services and the grooming school

·       Create strength-based goals via Strengths Assessment and tracks progress

·       Monitor outcomes for case management services

·       Teaches soft skills classes as needed

·       Serves up to 20 students, 100% programming.

Minimum Requirements:

·       Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.


·       Bachelor’s degree in a human service-related field

·       3+ years’ experience coaching low-income parents entering the job market with a proven track record for success

Working conditions:

·       Must be able to drive your own vehicle to appointments

·       Be comfortable working in a small office

·       Must be comfortable around dogs


Compensation: $42,760- $48,760
